Training Request Form

Once this form is filled out, you will be contacted to complete the registration process.  Be advised that funds are due PRIOR to the start of class.  Bringing funds the first day of class is acceptable. Once your form is successfully submitted, you will be automatically returned to the web site.  Should you have problems with this form, please try submitting your training request from a non-government computer, as many government computers have hightened security settings. If that does not work, please email dan@jsetusa.com or call us at 480-748-3992.  Thank you for using our online registration process.  If you receive an error message when attempting to send your registration and you do not have access to a home computer, you may fax the information required on the form to (623) 825-0264.  Please use this method of registration as a last resort.  Thank you!

Tuition for the class is $495 per person.  The Early Registration price (registration and payment completed a minimum of two weeks prior to the training date) is $450.  Once you have completed the registration form, feel free to go to our Online Store, where you can pay utilizing our secure site.  ALL fees are due by the start of the first day of class. 

NOTE: You must be a sworn officer or reserve to attend this training.    Civilian employees of police departments who have gone through a background check MAY be accepted on a case-by-case basis.  Sorry, NO private security will be allowed.

Training Request Form

IMPORTANT NOTICE

Please be advised that ALL funds for training are due by no later than the start of class.  No funds or proof of payment, no class.  Per the City of Phoenix Personnel Department, Phoenix Officers may not use Blue Card money to pay for this class at this time.  This is THEIR decision, not ours.  If this policy changes we will be happy to accept Blue Card funds.  Sorry for any inconvenience.


Failure to show (FTA) results in the student or department being charged the FULL tuition price for the class.  Unless an emergency arises, notification must be made a minimum of three full days prior to the date of the class.

It is the student's responsibility to ensure that payment requests are submitted through their department. In most cases that requires submitting paper work at least 30 days before the course begins. As an alternative, you may pay with personal funds and request departmental reimbursement following the training.  Remember to submit department paperwork early enough to take advantage of the Early Registration rate of $450.
